Resumo Déficits cognitivos são comuns em pacientes após acidente vascular cerebral (AVC). O prejuízo cognitivo causado por esse evento é mediado por variáveis etárias e de escolaridade. No Brasil, o único instrumento de rastreio cognitivo específico para o pós-AVC é a Triagem Cognitiva (TRIACOG). O objetivo deste estudo é investigar evidências de validade relacionadas a variáveis externas da TRIACOG. Participaram do estudo 153 adultos e idosos (M = 60,08; DP = 9,61) de Porto Alegre e região metropolitana, sendo 87 pacientes pós-AVC e 66 saudáveis. Three-way ANOVA foi utilizada para indicar os efeitos e interações entre variáveis de grupo, etárias e educacionais. Observou-se a influência dos fatores de grupo e idade nos escores da TRIACOG. Ressalta-se a relevância dos resultados para a construção de pontos de corte para tarefas e funções do instrumento, considerando aspectos educacionais e etários, aumentando a precisão na identificação de déficits em pacientes pós-AVC.

Abstract Cognitive deficits are common among post-stroke patients. Cognitive impairments of this sort are mediated by age and education. In Brazil, the only specific cognitive screening tool designed for post-stroke patients is the Cognitive Screening Test (Triagem Cognitiva - TRIACOG). The goal of this study was to investigate validity evidence related to external variables for the TRIACOG. Our sample included 153 adults and elderly people (M = 60.08, SD = 9.61) from Porto Alegre and metropolitan area, comprising 87 post-stroke patients and 66 healthy individuals. Three-way ANOVAs were used to assess main effects and interactions between the variables group (clinical/control), age and education. An influence of group and age on scores in the TRIACOG was found. We emphasize the relevance of these results to the selection of cut-off points for the tasks and cognitive functions assessed by the instrument, considering education and age, so as to allow more accurate identification of deficits in post-stroke patients.

Resumen Los déficits cognitivos son comunes en pacientes después de un accidente cerebrovascular. El deterioro cognitivo causado por este evento está mediado por variables de edad y educación. En Brasil, la única herramienta de detección cognitiva específica para después de un accidente cerebrovascular es el Cribado de Deterioro Cognitivo (TRIACOG). El propósito de este estudio fue investigar evidencias de validez relacionadas con las variables externas de TRIACOG. Participaron en el estudio un total de 153 adultos y ancianos (M = 60.08; DS= 9.61) de Porto Alegre y región metropolitana, de los cuales, 87 eran pacientes posictus y 66 eran sanos. Se utilizó Three-way ANOVA para indicar los efectos y las interacciones entre las variables de grupo, edad y escolarización. Se observó la influencia de factores de grupo y edad en las puntuaciones del TRIACOG. Se enfatiza la relevancia de los resultados para la construcción de puntos de corte para tareas y funciones del instrumento, teniendo en cuenta aspectos educativos y de edad, aumentando la precisión en la identificación de déficits en pacientes posictus.

RESUMO. Instrumentos de triagem são ideias no contexto clínico hospitalar, uma vez que são fáceis de administrar, rápidos, tem baixo custo e são sensíveis para amostras específicas. Portanto, há a necessidade de se verificar as propriedades psicométricas de instrumentos de triagem para pacientes pós acidente vascular cerebral. Objetivo: Este estudo investigou as propriedades psicométricas (procedimentos metodológicos) de triagens cognitivas para pacientes com doenças cerebrovasculares. Métodos: Foi realizada uma revisão sistemática de artigos publicados em PsycINFO, Web of Knowledge, PubMed e Science Direct (2005 a 2016). Apenas 55 artigos permaneceram após a aplicação dos critérios de exclusão. Resultados: As amostras variaram de 20 a 657 pacientes; a maioria dos artigos avaliou indivíduos idosos, com quatro a 13 anos de educação, que sofreram AVC isquêmico e hemorrágico. Houve uma tendência para encontrar evidências de validade relacionadas ao critério e analisar a sensibilidade/especificidade dos instrumentos. Embora os estudos frequentemente apliquem o Mini Exame do Estado Mental (MMSE) e o Montreal Cognitive Assessment (MoCA) para buscar evidências de validade e de fidedignidade, o uso desses instrumentos em pacientes com AVC é criticado por razões relacionadas às suas propriedades psicométricas e funções neuropsicológicas avaliadas. Conclusão: Embora não exista uma triagem padrão-ouro para avaliação de adultos pós-AVC, os instrumentos construídos especificamente para esta população mostraram-se promissores. Este estudo de revisão contribui tanto aos pesquisadores quanto aos clínicos que desejam selecionar o rastreio mais apropriado para identificar comprometimento cognitivo em adultos pós-AVC.

ABSTRACT. Screening instruments are ideal for acute clinical settings because they are easy to apply, fast, inexpensive and sensitive for specific samples. However, there is a need to verify the psychometric properties of screening in stroke patients. Objective: This study investigated the psychometric properties (methodological procedures) of cognitive screening for patients with cerebrovascular diseases. Methods: A systematic review of papers published on PsycINFO, Web of Knowledge, PubMed and Science Direct (2005 to 2016) was performed. Results: A total of 55 articles remained after applying exclusion criteria. The samples ranged from 20 to 657 patients. Most articles evaluated elderly individuals with four to 13 years of education who had experienced ischemic or hemorrhagic stroke. There was a tendency to find evidence of validity for criteria and to analyze the sensitivity/specificity of the instruments. Although the studies frequently used the Mini-Mental State Examination (MMSE) and the Montreal Cognitive Assessment (MoCA) to seek evidence of validity and reliability, the use of these instruments among stroke patients has been criticized due to their psychometric properties and the neuropsychological functions evaluated. Conclusion: Although there is no gold standard screen for assessing adults post-stroke, instruments devised specifically for this population have shown promise. This review helps both researchers and clinicians to select the most appropriate screen for identifying cognitive impairment in adults post-stroke.

Resumo Neste estudo, investigaram-se efeitos de idade e escolaridade e suas interações no desempenho de adultos no Instrumento de Avaliação Neuropsicológica Breve NEUPSILIN, que avalia orientação têmporo-espacial, atenção, percepção, memória, linguagem, calculias, praxias e funções executivas. Participaram 627 adultos saudáveis (19 a 90 anos), divididos em três grupos, conforme escolaridade (1 a 4, 5 a 8 e 9 anos ou mais de estudo) e em quatro grupos por idade (19 a 39, 40 a 59, 60 a 75 e 76 a 90 anos). Encontraram-se efeitos de idade e escolaridade em atenção, percepção, memória (de trabalho, verbal episódica, semântica, visual e prospectiva), linguagem oral e escrita, praxias e funções executivas. Interações entre idade e escolaridade foram encontradas em atenção, memória verbal, linguagem oral e escrita. Os resultados destacam que o envelhecimento e a escolaridade impactam de forma heterogênea nas funções cognitivas, assim como os testes neuropsicológicos brasileiros devem sempre considerar a influência dessas variáveis para produzir seus dados normativos.

Abstract This study investigated the effects of age and schooling and their interactions on the performance of adults in the NEUPSILIN Brief Neuropsychological Assessment Instrument, which evaluates time and space orientation, attention, perception, memory, language, calculation, motor functions, and executive functions. The sample was composed of 627 healthy adults (19 to 90 years), who were divided into three groups according to education range (1 to 4, 5 to 8 and 9 or more years of study) and into four groups by age range (19-39, 40-59, 60-75 and 76-90 years). We identified age and schooling effects in attention, perception, memory (working, verbal episodic, semantic, visual and prospective memory), oral and written language, motor functions, and executive functions. Interactions between age and schooling were observed in attention, verbal memory, and oral and written language tasks. The results highlighted that aging and schooling impact cognitive functions heterogeneously, and that Brazilian neuropsychological tests should always consider the influence of these variables to produce their normative data.

Resumen En este estudio se investigaron los efectos de edad y escolaridad, y las interacciones en el desempeño de adultos en el Instrumento de Evaluación Neuropsicológica Breve NEUPSILIN que evalúa la orientación espacio-temporal, atención, percepción, memoria, lenguaje, cálculos, prácticas y funciones ejecutivas. Participaron 627 adultos que gozaban de buena salud (19-90 años), divididos en tres grupos según el nivel educativo (1 a 4, 5 a 8 y 9 o más años de estudio) y cuatro grupos por edad (19 a 39, 40 a 59, 60 a 75 y 76 a 90 años). Se encontraron efectos de la edad y de educación en atención, percepción, memoria (de trabajo, episódica, semántica, visual y futura) lenguaje, oral y escrita, prácticas y funciones ejecutivas. Interacciones entre edad y escolaridad fueron encontradas en atención, memoria verbal, lenguaje oral y escrita. Los resultados destacan que el envejecimiento y la escolaridad impactan de forma heterogénea en las funciones cognitivas, y que los tests neuropsicológicos brasileños deben considerar siempre la influencia de estas variables para producir datos normativos.

OBJETIVO:Investigar aspectos preservados e dificuldades na escrita de palavras e pseudopalavras em adultos que sofreram acidente vascular cerebral (AVC) à esquerda e à direita e discutir os perfis de disgrafia adquirida nesses indivíduos.MÉTODOS: Investigaram-se perfis de disgrafia adquirida a partir da avaliação das habilidades e dificuldades na escrita de palavras e pseudopalavras de seis adultos que sofreram AVC no hemisfério direito (LHD) e no hemisfério esquerdo (LHE), comparados a adultos neurologicamente saudáveis. Realizou-se análise de séries de casos com os pacientes que apresentaram desempenho deficitário na escrita de palavras, que indicavam a presença de uma disgrafia adquirida.RESULTADOS: Foram identificados dois casos com disgrafia lexical (sendo um com LHE e outro com LHD), um caso com disgrafia fonológica, um com disgrafia periférica, um com disgrafia mista e um com disgrafia por déficit no buffergrafêmico, todos estes com LHE. Destacou-se nesse estudo a heterogeneidade das habilidades linguísticas dos casos clínicos, discutidas de acordo com o modelo cognitivo de dupla-rota de escrita.CONCLUSÃO: O maior prejuízo encontrado nos pacientes com LHE ressalta a importância desse hemisfério cerebral para o processamento da escrita de palavras. A presença de um caso com LHD com perfil de disgrafia lexical destaca a necessidade de melhor estudar o papel do hemisfério direito no processamento de palavras. Espera-se que esse estudo contribua para o planejamento de estratégias de intervenção neuropsicológica direcionadas à escrita de palavras.

OBJECTIVE: This study aimed to assess the strengths and difficulties in word and pseudoword writing in adults with left- and right-hemisphere strokes, and discuss the profiles of acquired dysgraphia in these individuals.METHODS: The profiles of six adults with acquired dysgraphia in left- or right-hemisphere strokes were investigated by comparing their performance on word and pseudoword writing tasks against that of neurologically healthy adults. A case series analysis was performed on the patients whose impairments on the task were indicative of acquired dysgraphia.RESULTS: Two patients were diagnosed with lexical dysgraphia (one with left hemisphere damage, and the other with right hemisphere damage), one with phonological dysgraphia, another patient with peripheral dysgraphia, one patient with mixed dysgraphia and the last with dysgraphia due to damage to the graphemic buffer. The latter patients all had left-hemisphere damage (LHD). The patterns of impairment observed in each patient were discussed based on the dual-route model of writing.CONCLUSION: The fact that most patients had LHD rather than right-hemisphere damage (RHD) highlights the importance of the former structure for word processing. However, the fact that lexical dysgraphia was also diagnosed in a patient with RHD suggests that these individuals may develop writing impairments due to damage to the lexical route, leading to heavier reliance on phonological processing. Our results are of significant importance to the planning of writing interventions in neuropsychology.

The aim of the present study was to review the tasks that have been used to assess the functioning of the episodic buffer in Baddeley's multicomponent model of working memory. A systematic review of studies published from January 2000 to February 2013 was conducted. The search term "episodic buffer" was used in the Web of Knowledge, PsycINFO, PubMed, Embase, and BVS-Psi databases. The selected articles consisted of empirical studies that used tasks to assess the episodic buffer. Theoretical and review papers and studies with animals were excluded. The final sample comprised 36 papers. The tasks were categorized as experimental tasks or standardized tests. The experimental tasks were grouped by modality (unimodal or crossmodal) and described according to four criteria: task to be performed, type of stimulus used, secondary task employed, if any, and retention interval. The standardized tests included classical measures of working memory. Some tasks were found not to meet experimental criteria that were needed to evaluate the episodic buffer. Moreover, some of the standardized tests did not provide theoretical arguments or empirical evidence that the episodic buffer is recruited to perform them. The results are discussed in the context of the multicomponent model of working memory.

RESUMO Objetivo: Revisar as características metodológicas dos estudos sobre a reabilitação da afasia expressiva, descrevendo as técnicas de reabilitação utilizadas. Métodos: Foram pesquisados artigos nas bases de dados Medline, Science Direct e PubMed (Janeiro de 1999 a Dezembro de 2011), utilizando as palavras-chave Expressive / Broca / Nonfluent Aphasia, combinado com Language or Speech Rehabilitation / Therapy / Intervention. Resultados: Foram encontrados 56 artigos descrevendo técnicas de reabilitação, incluindo 22 com foco no processamento lexical, 18 na estimulação da sintaxe, sete com objetivo de desenvolver a fala e nove com múltiplos focos. Conclusão: Há variedade de técnicas e abordagens teóricas, destacando a heterogeneidade da investigação nesta área, que pode justificar-se pela singularidade dos déficits linguísticos dos pacientes, tornando-se difícil a generalização. Existe também necessidade de combinar as medidas formais de testes com medidas de habilidades pragmáticas e sociais da comunicação, para determinar o efeito da reabilitação na vida diária do paciente.

ABSTRACT Objective: This paper reviews the methodological characteristics of studies on rehabilitation of expressive aphasia, describing the techniques of rehabilitation used. Methods: The databases Medline, Science Direct and PubMed were searched for relevant articles (January 1999 to December 2011) using the keywords Expressive / Broca / Nonfluent Aphasia, combined with Language or Speech Rehabilitation / Therapy / Intervention. Results: A total of 56 articles were retrieved describing rehabilitation techniques, including 22 with a focus on lexical processing, 18 on syntax stimulation, seven with the aim of developing speech and nine with multiple foci. Conclusion: A variety of techniques and theoretical approaches are available, highlighting the heterogeneity of research in this area. This diversity can be justified by the uniqueness of patients' language deficits, making it difficult to generalize. In addition, there is a need to combine the formal measures of tests with measures of pragmatic and social skills of communication to determine the effect of rehabilitation on the patient's daily life.

A convencionalidade é um princípio pragmático da aquisição lexical que possibilita a adequação das palavras ao contexto. O objetivo deste estudo foi comparar dois tipos de análise - dicotômica e contínua - da convencionalidade dos verbos, obtidos por meio de uma tarefa de nomeação de ações composta por 17 vídeos breves, aplicada a 37 crianças com idades entre 2:0 e 3:0 e 43 crianças entre 3:1 e 4:5. Foram comparadas duas análises: (1) dicotômica - verbos classificados como não convencionais ou convencionais; e (2) contínua - escores entre 1 e 5, obtidos por escala likert. Os resultados mostraram que as duas formas de análise diferenciam os grupos etários, embora a análise contínua apresente vantagens, revelando mais detalhes do desenvolvimento do léxico de verbos.

Conventionality is a pragmatic principle of lexical acquisition which makes the adequacy of words to the context possible. The aim of this study was to compare dichotomic and continuous analysis in the classification of this pragmatic principle. The data were obtained by an actions naming task composed of 17 brief videos, applied to 37 children aged between 2:0 and 3:0 and 43 children aged between 3:1 and 4:5. Two analyses were compared: (1) dichotomic - verbs classified as non conventional or conventional; and (2) continuous, scores between 1 and 5, obtained using a Likert scale. The results showed that the two kinds of analysis distinguish the age groups, although the continuous analysis presents advantages, revealing more details about the verbs lexical development.
